Class Participation Waiver

I expressly release facility staff, class instructors and other participants from any and all liability for any serious injury or harm that may result from participation in class activities on behalf of myself and/or my child. I understand that Capoeira is a martial art and as such, classes may involve striking, throwing, punching, kicking, grappling, etc. which may result in various types of injuries. I understand that classes involve intense, strenuous physical activity and I assume all risks that may result from my participation or that of my child.Occasionally there may be pictures/videos taken to document possibly promote the classes

By signing below I acknowledge that I or my child (if applicable), am in good physical condition and have no serious medical issues that may preclude me from participating in physical activities with others or that would pose a health risk to those around me.

I HAVE READ AND UNDERSTAND THE ABOVE WAIVER AND RELEASE OF LIABILITY AND AGREE TO PARTICIPATE IN ALL CLASS ACTIVITIES AND DO HEREBY SIGN VOLUNTARILY.
